About the McCormick Transitional Care Center

The 40-bed Timothy R. McCormick Transitional Care Center, named after retiring president and CEO of Unity Health System, offers short-term care following hospitalization so patients can recover before returning to their own homes. 

Our patient-directed philosophy of care is simple: we're focused on you. Our team will work with you and your hospital medical team to develop a plan of treatment based on your unique recovery needs.

All rooms in our Transitional Care Center are private and have a view of the beautiful courtyard. Each room also has a private bath and a flat-screen TV. Meals are served buffet-style in the spacious dining room and smaller lounges are located throughout the building to provide comfortable spaces for conversation and relaxation. Computer and wireless internet access is available.

We provide nursing, medical and rehabilitation services including:

  • Joint replacement and complex fracture recovery
  • Stroke and neurological rehabilitation
  • Physical, occupational and speech therapy
  • Nutritional counseling
  • Dialysis services
  • Cardiac rehabilitation
  • Chemotherapy
  • Pain management
  • Intravenous therapy

  • Discharge planning

Admissions

Thank you for your interest in Rochester Regional's McCormick Transitional Care Center facility. In order to be considered for admission, an application must be completed and returned to the long-term care admissions office.
