Due to high patient volume, it is important that you arrive to your appointment on time (or early). If you are 10 minutes (or more) late, you will have to wait for the next available provider. If a provider is not available, your appointment may have to be rescheduled for a different day.
Our center is staffed by full-time Social Workers, Care Managers, Mental Health Therapist and Financial Case Managers to provide one-on-one services for our patients. Spanish and sign language interpreters are also available.
Patient-Centered Medical Home (PCMH) is a nationally recognized program that brings patients together with doctors, nurses, and other members of the care team to focus on helping patients get healthy and stay healthy every day.
