Rochester Regional Health and ElderONE Celebrate Helping More Seniors Live at Home

As Rochester Regional Health and members of the community celebrate the new ElderONE center at 800 Emerson Street, program participant Lillie Mae Chambers looks around and simply states, “it’s just so beautiful, and a joy to be around here.”

ElderONE is a Program of All-Inclusive Care for the Elderly (PACE) that allows older adults to continue to live in their own homes. Through three locations, the program provides 680 participants with integrated medical care, home care and care at the center during the daytime. With a team that includes healthcare providers, physical, occupational and recreation therapists, dietitians, social workers and transportation services, ElderONE combines the support and peace of mind seniors and their loved ones need with the independence many are reluctant to let go.

Chambers is one of more than 160 seniors who now participate at the new Emerson Street ElderONE location. “Since coming here, Lillie Mae has become a pillar of the ElderONE community, joining committees, welcoming new participants and spreading her infectious positive outlook and gratitude,” remarked Jenn Woltman, senior PACE manager for the ElderONE Emerson Center.

The grandmother of 24 is a retired nursing aide and housekeeper. In fall 2015, Chambers’ doctor recommended she enroll in ElderONE to receive help with day-to-day tasks, healthcare, physical activity and socialization. In this new setting, she thrived. “Physically, mentally, I feel so much better.”

“ElderONE is here to help frail seniors live in their own homes for as long as possible,” said Rochester Regional Health Senior Vice President for Long Term Care Kathy McGuire.

Without ElderONE, Lillie Mae believes she “would probably be living in a nursing home or a hospital and be halfway gone.” “But now,” she says with a smile, “I could go on forever.”

The center at 800 Emerson Street is the program’s third. The other two centers are located at 2066 Hudson Avenue and 355 North Park Drive in Rochester.
