Notice to Our Patients of an Email Incident

We value the privacy and security of our patients’ information. Regrettably, this notice is to inform our patients of a recent incident that may have involved some patients’ information.

On April 2, 2020, we learned that there may have been unauthorized access to an employee’s email account between January 13, 2020, and January 23, 2020. We quickly secured the email account, began an investigation, and a cyber security firm was engaged to assist. Our investigation determined that the information contained in the email account may have included some patients’ names, dates of birth, medical record numbers, and limited clinical information related to the care patients received at Unity Behavioral Health. At this time, we do not believe any social security numbers were impacted as a result of this incident.

We have no indication that any patient’s information has been misused. However, in an abundance of caution, we began mailing letters to affected patients on June 1, 2020, and established a dedicated call center to answer any questions our patients may have. If you believe you are affected and do not receive a letter by July 1, 2020, please call 1-844-989-2773, Monday through Friday, between 9:00 a.m. and 6:30 p.m. Eastern Time.

We recommend affected patients review the statements they receive from their healthcare providers. If they see any discrepancies or services they did not receive, they should contact the provider immediately.

We deeply regret any concern or inconvenience this incident may cause our patients. We are reinforcing information security procedures with our employees and implementing changes to help prevent an incident like this from happening again.
