About the Clifton Springs Diabetes Health Center
The Clifton Springs Diabetes Health Center provides outpatient diabetes educational programs to residents of the Finger Lakes region. The Clifton Springs Hospital Diabetes Self-Management Education Program has been Recognized by the American Diabetes Association for providing Quality Self-Management Education* in accordance with the National Standards for Diabetes Patient Education Programs. We are also affiliated with Healthways, Inc., the country’s largest provider of specialized diabetes services.
We've made it our mission to provide comprehensive diabetes self-management education that is state-of-the-art. A variety of outpatient educational programs and services are offered. A referral form completed by your healthcare provider is required before registering for any class. Classes include:
- Core Curriculum Class (7.5 to 10 hours)
Gain a strong foundation of knowledge and skills. Designed for individuals new to diabetes, with pre-existing diabetes and need a review, or are having difficulty with daily self-management or control of their diabetes. Morning, afternoon, evening, and all-day sessions are available.
- Injectable Medication—Insulin, Byetta, Symlin (1 to 1.5 hours)
Whether new to insulin or needing a review, this class will help ease any apprehension you may have by learning proper injection techniques and other useful information regarding injectable medications.
- Pre-Diabetes / Impaired Glucose Tolerance (1.5 hours)
Diagnosed with pre-diabetes? Take action now and learn how to reduce and control your blood glucose levels and minimize your risk of health complications later.
- Insulin Pump Education (1 to 6.5 hours)
Planning on going on a pump? This comprehensive instruction provides information to help you be confident and successful with your pump therapy. For those currently on pumps, evaluation and assistance with fine-tuning is available.
- Advanced Carbohydrate Counting (1.5 hours)
Take an in-depth look at the art and science of counting carbohydrates (“carbs”), making healthy and appropriate dietary changes, and matching insulin dosing to carbohydrate intake.
- Gestational Diabetes Mellitus (3 hours)
Helping mom and baby stay healthy is our key focus. This class and follow-up session assist women who are experiencing high blood glucose levels during pregnancy. Additionally, we offer a complimentary one-hour session at 6-weeks postpartum to help mom learn how to reduce her risk of developing type 2 diabetes.
- Individual Counseling Sessions
We recommend attending our classes, but we know the classroom setting is not always appropriate. Individual appointments with our registered dietitian and registered nurses are available, if needed, and may include follow-up sessions.
