    Our providers offer longitudinal primary care of home-bound patients with chronic complex medical, social, and mental health conditions, particularly those at high-risk of hospital, nursing home, or recurrent emergency room care.

    Dr. Karlic and Dr. Son are geriatric physicians that cover our patients when they are admitted to Unity Hospital. Our physicians also provide consult services for the Geriatric Surgical Verification program for hospitalized surgical patients who meet criteria.
