Rochester Regional Health geriatric medicine physicians and advanced practice providers work in multiple settings including the office, independent living facilities, assisted living facilities, nursing homes, and in the hospital.
Our physicians provide primary care services to patients above the age of 55. Our office resources include a warfarin nurse, social worker support, and a health care coach.
Dr. Khan provides longitudinal primary care of home-bound patients with chronic complex medical, social, and mental health conditions, particularly those at high-risk of hospital, nursing home, or recurrent emergency room care. Some criteria include: home-bound status due to medical or mental health conditions, residence within a reasonable driving radius from Unity Geriatric Associates primary office, the goals of care must be comfort care, and non-smoking household.
Unity Geriatric Associates provides medical services to a number of nursing homes, assisted living facilities and independent living facilities in our community. Our physicians also act as medical directors to a number of these locations.
