The Cardiac Rehabilitation Center at Clifton Springs Hospital & Clinic offers a three-phase approach to strengthening your heart and promoting your health. This program is beneficial for individuals who have had bypass surgery, a heart attack, angioplasty, atherectomy, angina, have known heart disease or need risk factor modification.
The program is designed to enhance an individual's health and wellness. This is facilitated by a multi-disciplinary team that focuses on:
- risk factor modification
- stress management
- weight loss
- smoking cessation
Phase 1 takes place during the first few days of your hospitalization. During your stay, a member of the cardiac rehabilitation team will speak with you regarding your heart and risk factors for heart disease. This is the beginning of an educational process to help you achieve your goals of striving for health and happiness. After being discharged from the hospital, you can begin an exercise program by participating in Phase 2.
Phase 2 Phase 2 is a six to 18 week outpatient program designed to offer information, education, exercise, and support for dealing with your illness. You can begin Phase 2 once you have received a referral from your physician and an exercise tolerance test has been completed. A medically appropriate individualized exercise program will then be developed. During exercise, your heart will be monitored and you will be supervised by a member of the rehabilitation team. In addition to exercise, education classes on such topics as stress management, risk factor modification, smoking cessation, and diet are offered.
Phase 2 is covered by most insurance policies. The billing office at Clifton Springs Hospital & Clinic will be happy to check into your policy coverage for you. Upon completion of Phase 2, a maintenance program is recommended.
Phase 3 is designed for individuals who wish to continue their exercise program under medical supervision and for individuals who need risk factor modification. Many individuals continue their exercise program at Clifton Springs Hospital & Clinic and enjoy the benefits and comforts of a medically supervised program and the camaraderie of a supportive environment. Phase 3 is not covered by insurance so individuals can participate on a self-pay basis as often as they desire.
