What to Bring to Your First Visit

Please arrive at least 15 minutes before your appointment to check-in and fill out forms and bring the following:

  • Photo I.D.
  • Health insurance card
  • Insurance authorization, if required
  • Doctor's referral, if required
  • Recent test results related to your condition
  • A list of your medications, including nutritional supplements, and their dosages
  • A list of medications you are allergic to, if applicable
  • A list of questions you would like to ask the physician
  • Device or paper for taking notes
