About Children's Health Home Care Management
This program is designed to help children who may have ongoing chronic health issues, behavioral health needs, and/or addiction treatment needs. The healthcare and service providers at Health Home Care Management will help you receive the services you need.
All children/youth must meet the following eligibility requirements to be considered for enrollment:
The child/youth is under the age of 21, enrolled in Medicaid and resides in Monroe county.
The child/youth must meet the NYS DOH eligibility
Child/youth has significant behavioral, medical or social risk factors which can be addressed through care management
We can help if you experience any of the following:
- a chemical addiction condition
- a mental health condition
- a chronic physical condition
- difficulty finding a provider or getting to your appointments
- frequent visits to the ER
- unsafe living conditions
