Community Outreach Programs
We offer many community-based outreach programs for the vulnerable and at-risk residents in the communities we serve. All of our services are provided in a caring and compassionate manner. These programs offer a variety of services to individuals, including primary care and dental care, referrals, education, nutrition counseling, prenatal services, job training, and housing. Community Impact
We are participating in a national initiative - Partnership for Patients (PFP) - aimed at improving the quality of care and making healthcare safer by reducing hospital-acquired conditions and re-admissions. NYS Partnership for Patients was recognized as one of the nation's highest-performing programs and was highlighted by a U.S. Department of Health and Human Services report in 2014.
