Primary In-Home Services

With the help of Rochester Regional Health Home Care, patients can receive skilled professional care and assistance with activities of daily living (ADLs). 

Skilled Nursing

Health care professionals with advanced education and training offer services including:

Skilled nursing is care that can only be performed by licensed nurses and is regulated by the U.S. Department of Health. Our skilled nurses offers you any teaching or training that is specific to your health condition(s), self-care techniques, and medication management.

Rehab Therapies & Care

Rehabilitation specialists involve you and your family/caregivers to set goals for treatment from a temporary disability or surgery, like a joint replacement. Rehabilitation services include:

  • Multi-disciplinary rehab care
  • Physical therapy
  • Occupational therapy
  • Speech/language pathology
  • Case management
  • Fall prevention/safety assessment

Our specially trained rehabilitation specialists will work with you and help you learn, re-learn, or engage in a full spectrum of activities.

Medical Social Work

Through our medical social work services, we assist patients and their families in dealing with financial issues and help connect them with helpful community resources. Some ways we assist include:

  • Financial counseling and advocacy
  • Emotional support
  • Long-term care needs
  • Linking patients & families with community services

Our medical social workers primarily focus on supporting patients and their families by coordinating care with the larger medical team. They play an essential role in many of the non-medical aspects of patient care.

Home Infusion Therapy

If you’d like to avoid doctor or clinic and hospital visits, we offer home infusion therapies in the comfort of your home. Our state-of-the-art technology and specially trained nurses can administer your treatment without you leaving your house.

Our home infusion therapy offers 24-hour clinical support for patients, utilizing user-friendly therapies like IV antibiotics, hydration, total parenteral nutrition, pain medication, and chemotherapy.

Home Health Aides

Our home health aides are here to assist you with activities of daily living. Those may include:

  • Getting in and out of bed
  • Bathing and dressing
  • Going to the restroom
  • Walking
  • Preparing meals and eating

Home health aides do not provide services like physical or occupational therapy or skilled nursing care, but they are often tasked with observing the patient’s physical and mental health. These observations are reported and can help inform care decisions. Our compassionate home health aides are an integral part of our home health team.

Specialty In-Home Services

Our experienced caregivers offer advanced, comprehensive services for patients with chronic conditions who need special care. Whether you are recovering from a major surgery, are currently receiving cancer treatments, or are suffering from a disease like Parkinson’s, our team of experts will coordinate personalized specialty care.

Cardiopulmonary Care

Our team of nurses is here to help you manage your heart disease. Our Nurse Specialists and specially-trained RNs assess, train, and teach care techniques. We’re also on-hand to help with case management, interdisciplinary coordination, and timely MD notification.                        

Diabetes Care

There aren’t treatments for diabetes, but we can educate and care for anyone by developing a manageable treatment plan. Our certified Diabetes Nurse educators and specially trained team of nurses instruct newly diagnosed patients in blood glucose monitoring, meal planning, exercise, and medications.

We offer personalized care planning for chronic diabetic self-management, including glycemic control, adaptive devices, and coping strategies.

Cancer Care

Cancer care isn’t just about treatment, it also involves managing pain and the side effects of your treatment. Our specially trained nurses offer medical and post-surgical care, assessment, treatment, and teaching.

They specialize in post-chemotherapy and post-radiation care, can help with at-home chemotherapy infusion, and identify long-term care needs. If you or a loved one need case management support or home cancer care, we are here to help.

Parkinson’s Care

Our interdisciplinary team, trained through the National Parkinson Foundation, collaborates with patients, family, and the health care team to offer evidence-based, targeted therapy interventions. Our Parkinson’s Disease education and connection to community resources allow us to offer thorough, compassionate care.

Our Home Health Care team includes specialists with national certification in wound, ostomy, and continence nursing. We offer chronic wound management using evidence-based guidelines, including vacuum assisted wound closure (VAC) and ostomy, fistula care, and drainage management.
