To be considered for admission in one of our long term care facilities, an application must be completed and returned to the long-term care admissions office. Print, complete, and return the forms below, along with the New York State required PRI and SCREEN, to the admissions office.
To assist families in their decision on which long term care center will best serve their needs, we offer in-person tours at all six of our facilities, and our helpful admissions staff is happy to answer any additional questions you may have about the services offered at each. When you have made your final decision and would like to begin the admissions process for any one of our facilities, our staff will provide you with the admissions agreement tailored to the location of your choice.

To best serve our patients, Rochester Regional Health accepts a variety of payments for our long-term care services, including private payment, health insurance, long-term care insurance, Medicare, and Medicaid.
Rochester Regional Health provides residents and their families with the services of Long Term Care Medicaid insurance enrollment through our Financial Case Managers. Their role is to both help residents understand the Medicaid enrollment process and to partner with them and their families to attain the coverage needed for their stay.
Our Financial Case Managers have a vast knowledge of New York State Long Term Care Medicaid requirements. They will act as your advocate with the local Medicaid office, will assist with application questions, track your application through the system and keep you informed on the status.
