My Hospital Visit

Are you a patient at Unity Hospital? Use the My Hospital Visit portal to access some important information about your visit, including:

  • Current medical problems, allergies and immunizations
  • Physicians and specialists seen during your Hospital visit
  • Certain lab results during your Hospital visit
  • Discharge instructions
  • Medications prescribed at discharge
  • Patient education materials

If you need assistance logging into the My Hospital Visit portal, please call the helpline at 877.621.8014. If you have questions about your medical information in the portal, call the Unity Contact Center at 585.368.3000 for assistance between 8 a.m. and 6 p.m., Monday through Friday. My Doctor's Office

Are you a patient at a Unity doctor's office? Use the My Doctor's Office portal to:

  • Schedule or request non-urgent appointments

To setup your My Doctor's Office account, ask your doctor's office for an activation code (token). Once you have the code, click here and follow the instructions.

United Memorial Medical Center

Need to enroll?

Click here to Self-Enroll and fill out all of the required information and then select submit. You will be required to enter your Name, Date of Birth, a valid Email Address and your Medical Record Number or Social Security Number (all information must match what is on the file at United Memorial Medical Center).

Once you complete the enrollment process you will receive an email with a one-time User ID and Password that will give you access to the Patient Portal Site and you will then be prompted to create your own unique User ID and Password.

Clifton Springs Hospital & Clinic

All of the information in My Health Portal comes from your Clifton Springs Hospital & Clinic Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.

With My Health Portal you can:

  • Access parts of your health information online, including lab results, radiology reports, visit summaries
  • View appointments
  • View your bills online
  • View a family member's chart as a proxy
