Assertive Community Treatment (ACT) is often referred to as our treatment without walls. It is a community-based program that delivers comprehensive services to individuals diagnosed with serious mental illness whose needs have not been met by traditional, office-based services. It is an effective, evidence-based program highly regulated by New York State. The program requires six contacts per month with each patient, five of which must be local and face-to-face. Participation is voluntary, although many of these arrangements are part of court-ordered Assisted Outpatient Treatment (AOT) and average a five-year length of stay with the goal of transitioning back to a traditional clinic setting.
ACT services are designed to help participants achieve and maintain life goals such as:
· Stable housing
· Employment
· Education
· Symptom management
· Personal stability
· Improved health and wellness
· Reduction in medical and psychiatric hospitalizations
· Integration with the larger community
Our multidisciplinary team will work with you or your loved one to develop an individualized treatment plan. This plan works to meet your specific needs to allow a safe return home or to a less restrictive level of care, as soon as the patient is ready. We provide holistic care that focuses on ways to maintain good long-term mental health. Part of this goal includes offering education for both you and your family and financial assistance when needed.
