When you require hospitalization, your primary care physician may request that a specially trained physician, called a Hospitalist, oversees your care during your stay in the hospital. Hospitalists realize that your primary care physician's involvement in your care is important. That's why the Hospitalists take extra measures to stay in touch with your primary care doctor throughout the hospitalization.
A hospitalist is a physician who specializes in caring for patients in the hospital and who usually does not have an outpatient practice. They are internal medicine physicians who will be seeing you on my behalf while you are in the hospital.
This arrangement was made so you have the benefit of a Hospitalist physician (a doctor who is entirely focused on the care of hospitalized patients), and a medical team that is on site and always available to you and your family in case you have an urgent medical need. It also provides me additional time to see patients, including you, at my office.
Once your condition improves and your care continues at home, I will resume providing your care. Please call my office to schedule a follow-up appointment after you are discharged. The Hospitalist will promptly send your information to my office so that you will receive uninterrupted care.
Because we work solely in the hospital, they can see you more than once a day if needed. They can check on your test results in a timely manner to make adjustments in your treatment throughout the day, and are available to coordinate your care. They are generally able to schedule meetings at your convenience during regular business hours to answer questions that you and your family may have.
From the start of your admission, the Hospitalist and I will be communicating on a regular basis. At the time of your admission, records from my office are requested and your condition discussed. I will be kept informed about your condition as necessary throughout your stay.
Every effort will be made to have the same Hospitalist physician see you daily. Because they provide care 24 hours a day, seven days a week, there will be times when another doctor sees you. All those providing your care will be fully informed about your condition.
Our goal is to make your return to your home as smooth as possible. The Hospitalist team will work with me from the time of your admission to enable a timely and medically appropriate release from the hospital.
